SMC Patient Information
Save time during your first appointment by completing the Patient Information Form, Notice of Privacy Practices and Privacy Practices Acknowledgement Form (all listed on the right). Simply print out all forms, fill in the information requested, and bring them with you to your appointment.
Payments & Insurance
We accept Medicare, Medicaid, Workmen’s Compensation, Aetna, Tricare, and many other insurance. We are preferred providers with Blue Cross Blue Shield.
We have staff available to help make the billing and payment process as easy and convenient as possible. We will bill your insurance carrier directly. We strongly encourage that you review your benefits and coverage limitations to avoid financial surprises. Arrangements are available for those with limited or no insurance benefits.
